Tranny trouble

My 87 190 2.3 8 vlv is having trouble shifting properly. I have changed the fluid and filter (had a leaky pan). It had the same problem before the fluid change. The problem is it won't shift into 4th until appx 62 mph. It goes into gear without any delay, forward and reverse. Normally starts in second, shifts to 3rd just fine, just doesn't like to go into 4th. I've tried adjusting the bowden cable with no luck. The cable is as loose as it can get. If I completely remove the cable from the throttle linkage the car will shift through all of the gears very quickly, getting into 4th by 30mph. With the cable connected it'll shift up to 3rd normally and once it gets to about 40mph if I quickly shift to neutral and back to drive it will shift to 4th. Once in 4th it cruises along just fine. I can get it to shift at any speed by doing the neutral/drive thing. But it just won't do it on it's own until at least 62 or later depending on how hard I'm on the throttle. Anybody have any ideas what could cause this? Remember, this a 294K mile car! Any suggestions welcome. Thanks.
